BONE MARROW TRANSPLANT UNIT


Samitivej Srinakarin hospital was located since September 30’ 2004 until 2007, we have bone marrow transplant‘s patient over 10 cases. The program provides both Autologous and Allogeneic related and unrelated bone marrow / stem cell transplant for lymphoma, leukemia, thallasemia with patients ranging in age from infancy through adolescence to adult. The bone marrow transplant teams are highly qualified and experience in oncologists, hematologists and array of specialists in both adult and pediatric medicine, professionals nurses, physical therapists, pharmacists, nutritionists, and other who participate in all aspects of patient care.

At the beginning of 2007 Samitivej Srinakarin hospital expanded area of bone marrow transplant unit to 9 th floor; have two hepafilters with positive pressure room.
New structures are in place to ensure that the unit environment is highly protective against infection, a major complication for patients undergoing bone marrow transplants, patients undergoing high dose of chemotherapy, patients that have low immune/ low white blood cell.


BONE MARROW

Bone marrow is a spongy tissue found inside bones. The bone marrow in the breast bone, skull, hips, ribs and spine contains stem cells that product the body blood cells.

These blood cells include

  • White blood cells (leukocytes), which fight infection
  • Red blood cells (erythrocytes), which carry oxygen, to as well as remove waste products from organs and tissues.
  • Platelets , which enable the blood to clot.

BONE MARROW TRANSPLANTATION

Bone marrow transplantation (BMT) is a special therapy for patients with cancer or other diseases which affect the bone marrow. A bone marrow transplant involves taking cells that are normally found in the bone marrow (stem cells), filtering those cells, and giving them back either to the patient or to another person. The goal of BMT is to transfuse healthy bone marrow cells into a person after their own unhealthy bone marrow has been eliminated. BMT have been use to treat patients who diagnosed with leukemia, aplastic anemia, lymphomas such as Hodgkin’s disease, multiple myeloma, immune deficiency disorders and some solid tumors.


TYPE OF BONE MARROW TRANSPLANT
Autologousand Allogeneic are the two main types of stem cell transplantation

  • Autologous bone marrow transplant
    The donor is the patient him/herself. Stem cells are taken from the patient either by bone marrow harvest or apheresis (peripheral blood stem cells) and then given back to the patient after intensive treatment. Often the term "rescue" is used instead of "transplant."
  • Allogeneic bone marrow transplant
    The donor shares the same genetic type as the patient. Stem cells are taken either by bone marrow harvest or apheresis (peripheral blood stem cells) from a genetically-matched donor, usually a brother or sister. Other donors for allogeneic bone marrow transplants include the following.

PROCESS OF BONE MARROW TRANSPLANTION
  • High-dose treatment
    The first stage of the treatment destroys your own bone marrow completely. This is done with high doses of chemotherapy, often combined with radiotherapy. After this treatment the donated stem cells are given to you through a drip into the vein or through your central line.

    The new marrow or stem cells, known as the graft, take a few weeks to settle in your bone marrow and produce the blood cells you need. Because of your low immunity, certain precautions will be taken to protect you from infections until your white cell count has recovered. You will be looked after in a room on your own and may be given antibiotics to help to protect you against infection.
  • Graft versus host disease
    Your doctors and nurses will be watching you carefully, both during the transplant, and for some months afterwards, for any signs of the new marrow reacting against your own body tissue (this is called graft versus host disease – GvHD). This can occur up to six months after your transplant and can cause various problems including diarrhea, rashes and liver damage, but it does not mean that your transplant has not worked. Your doctor will prescribe drugs to help prevent the graft reacting to your body.
